Southside Community Health Services accepts self-pay, private insurance, Medicare, and MNsure products including Medical Assistance, Minnesota Care, and Qualified Health Plans (QHP). Southside has a Sliding Fee Discount Program for those who quality. No one is ever turned away due to lack of insurance or inability to pay.
How to Pay Your Bill
We offer services on a sliding fee scale based on federal poverty guidelines of family size and income. We ask patients to bring proof of income to their appointment to help determine whether they are eligible for the discount program.
Some insurers and health plans require patients to select a primary clinic in order for services to be covered. Please check with your health plan to ensure that your visit to Southside will be covered, and bring your insurance card to every visit.
